Important Notice – Insurance Change Affecting Care at OFMC

 Effective February 15, 2026

Ormond Family Medical Center (OFMC) has been notified by Advent Health / FHHS that our network agreement will terminate for certain insurance plans, including:

• Oscar
  
• Healthfirst
  
• Aetna (only group numbers beginning with 175)
  

This change affects patients currently receiving care through these plans.

1. Self-Pay Visits – $155 per visit. Some referrals or prior authorizations may be limited.
   
2. Out-of-Network Benefits – You may submit claims directly to your insurance; costs may be higher.
   
3. Transfer to an In-Network Provider – Our staff can help coordinate a smooth transition. This means you will have to transfer your care to a different office/provider
   
4. You can change to an insurance plan that is contracted in 2026 with OFMC. see our list at www.ormondfamilymedicalcenter.com
